If you would like a PDF copy with all of the documents together, please send an inquiry via email to childcarehelp@culpeperhumanservices.org–keep in mind that this version of the application will not have the W-9 section fillable. You may also pick up a copy at the Culpeper Human Services office. We have a table set out front with applications. If you are picking up an application after hours, please check our document holders located to the right-hand side of the door.
The application period for the Childcare/Workforce Relief Program is December 2, 2020, starting at 9:00am, through January 18, 2021, at 4:00pm (no later).
